Proof of COVID-19 Vaccination Required for Traveling Aboard

When visiting certain foreign counties, you may be required to show documents that verify that you have been fully vaccinated against COVID-19.

Please be informed that Hikone City issues a COVID-19 vaccine passport to its residents.

Eligibility

Those who are required to submit a COVID-19 vaccine passport to go overseas.
Those who have been vaccinated with vaccine tickets issued by Hikone.

Requirements

1. Application Form

Form in English (Excelファイル: 36.5KB)

2. A Photocopy of a Passport

*No COVID-19 vaccine passport can be issued if a passport had expired.

Be sure to show a valid passport when requesting Hikone City to issue its vaccine passport.

3. A Photocopy of a Vaccine Ticket

*If you are a healthcare worker, please submit your vaccination record.

4. A Photocopy of Identification

*Driver’s license, residence card, health insurance card, etc.

How to Apply

By Post

Please submit all the requirements (see above) to the address below.

After your application is screened and accepted, the Hikone city hall will mail your vaccine passport to your address.

Hikone Health Promotion Division

If you choose to apply for your vaccine passport in person, please come to the Hikone Health Promotion Division (2F, Kusunoki Center) with all required documents.

You application will be screened, and if accepted, you will receive your vaccine passport through the mail.

NOTES

*To avoid creating crowded settings, you are recommended to apply by post.

*It may take one week to four weeks the Hikone Health Promotion Division to issue a vaccine passport.

*Be sure to fill out an application form. If there is even one error in your application form, your application will not be accepted.
