Starting on April 5, vaccination tickets will be mailed out to people over age 65, with their inoculations slated to begin in mid-May. The remaining members of the public will receive their doses of the COVID-19 vaccine in different phases at different times.
It may take you more time to receive the COVID-19 vaccination tickets than others do, so we ask for your understanding and patience.
Further details on how to schedule your vaccination appointment will be provided on the official website of Hikone City, Hikone Facebook page, etc. at a later date.

What's Inside?
1 COVID-19 Vaccine Ticket
Tickets for two doses of the vaccine and COVID-19 vaccination record are combined into one paper card (see the image on the left). Please bring the vaccination card each time you receive the jab.
When scheduling your appointment, you will be asked to provide your vaccination number printed on the vaccination card.
2 PrevaccinationScreeningQuestionnaireforCOVID-19vaccine
Be sure to fill out your pre-vaccination questionnaire (expect your body temperature) prior to the day of your vaccine appointment.
*Please use a ball-point pen to fill out the questionnaire.
