Please be informed that vaccination tickets for the COVID-19 vaccination are set to be mailed out to all residents aged between 12 and 64 on July 2, Friday. It may take vaccination tickets more time to be delivered to some than other, depending on where you live within Hikone.

Further details on how to schedule your vaccination appointment will be provided on the official website of Hikone City, Hikone Facebook page, etc. at a later date.

What's Inside?

1 COVID-19 Vaccine Ticket

Tickets for two doses of the vaccine and COVID-19 vaccination record are combined into one paper card (see the image on the left). Please bring the vaccination card each time you receive the jab.

When scheduling your appointment, you will be asked to provide your vaccination number printed on the vaccination card.

2 Prevaccination Screening Questionnaire for COVID-19 vaccine

Be sure to fill out your pre-vaccination questionnaire (expect your body temperature) prior to the day of your vaccine appointment.

*Please use a ball-point pen to fill out the questionnaire.
